Pursuing Dual Passions IN MEDICINE: Dr. ABRAHAM Sheffield's Insights on ENT PRACTICE and Genetic Research
Dr. Abraham Sheffield, an esteemed otorhinolaryngologist (ENT) and geneticist, hails from the Midwest. He completed his undergraduate studies at Brigham Young University (BYU) before pursuing a medical degree and a PhD in genetics at the University of Iowa. Throughout his academic journey, Dr. Sheffield has emphasized the importance of balancing a demanding medical career with family commitments.
Dr. Sheffield embarked on a challenging combined MD-PhD program at the University of Iowa, a decision heavily influenced by his roots and strong connections in the region. He highlighted the significance of selecting the right mentor for the PhD portion, ultimately joining Dr. Richard Smith's lab, which focuses on hearing loss and genetics. This experience not only enriched his knowledge but also shaped his future career decisions.
Navigating the Medical Field
The residency application process, which Dr. Sheffield likens to National Signing Day in college athletics, brought its own set of challenges. He initially explored multiple specialties before committing to ENT, a decision shaped by his experiences and the mentorship he received. He acknowledges the crucial role of good mentors who support their mentees' best interests, even when difficult decisions arise.
Despite planning to enter general ENT practice with his wife, Dr. Sheffield discovered a passion for pediatric ENT during his fifth year of residency. An unexpected offer for a pediatric ENT fellowship led to sleepless nights of contemplation. Ultimately, he chose to pursue the fellowship, recognizing its potential to make him a better surgeon and keep academic options open.
Although initially leaning towards private practice, Dr. Sheffield saw the fellowship as an opportunity for further training and family support. His father, a pediatrician with a research background, provided guidance during this decision-making process, emphasizing the importance of patient interaction over research.
A Passion for Genetics and Hearing Loss
Dr. Sheffield's PhD in genetics, particularly in hearing loss, brought both excitement and frustration. Despite his father's influence in medicine, he realized his true passion lay in patient care rather than research. This realization steered him towards a clinical specialty, influenced by the mentorship of Dr. Richard Smith and the cutting-edge gene therapy projects in his lab.
Dr. Sheffield's interest in hearing loss extended to technological advancements, such as cochlear implants. These devices, first used in the early 1980s, have seen significant improvements. Looking forward, Dr. Sheffield is optimistic about the potential of gene therapy to restore natural hearing, though he acknowledges it may take another decade to achieve. Genetic testing for hearing loss can identify about 50% of genetic causes, with ongoing research aiming to improve accuracy. Dr. Sheffield also highlights the syndromic nature of some types of deafness, which affect multiple body systems, underscoring the importance of early detection and comprehensive testing.
Balancing Family Alongside Career
Dr. Sheffield's personal life, including raising twins and managing career transitions, has been shaped by the need for balance. Both he and his wife, an oral surgeon, have navigated the challenges of dual medical careers. Their move to Quincy, Illinois, to join a multi-specialty group allowed them to achieve a better work-life balance, benefiting their marriage and family life.
Throughout his career, Dr. Sheffield has relied on the guidance of mentors and the support of his wife. He stresses the importance of understanding and communication, both in professional and personal contexts. Regular dates and mindful conversations have helped them manage the pressures of parenthood and maintain a strong relationship.
Addressing Sleep Apnea
In his practice, Dr. Sheffield treats sleep disordered breathing, particularly obstructive sleep apnea. He explains that CPAP therapy is the standard treatment, but alternative options like oral appliances, positional therapy, and the Inspire implant are available for those who struggle with CPAP machines.
Dr. Sheffield collaborates with specialists to address weight management in sleep apnea patients, recognizing that effective treatment often requires a holistic approach. He emphasizes the importance of basic health practices, such as good sleep, nutrition, exercise, and avoiding smoking, alongside high-tech medical solutions.
Dr. Sheffield concludes with a light-hearted reflection on his personal interests, including his son's potential need for ear tubes and his enthusiasm for Iowa Hawkeyes sports. His journey, marked by dedication to both his family and his profession, serves as an inspiring example of balancing the demands of a medical career with personal fulfillment.
